Por qué es importante que el coronavirus esté cambiando y qué significa esto para la eficacia de la vacuna
Una nueva variante de SARS-CoV-2 se está propagando rápidamente en el Reino Unido, con más de 1.400 casos desde septiembre. El SARS-CoV-2, el virus que causa COVID-19, generalmente acumula mutaciones lentamente con el tiempo, pero esta nueva variante ha acumulado muchas mutaciones rápidamente.
Si esta nueva versión del virus llegó para quedarse, como parece, ¿qué significa eso? ¿Esta nueva versión del virus reemplazará a la anterior? ¿Será más fácil de atrapar? Y, lo más importante, ¿seguirán siendo eficaces las vacunas actuales?
Esto me interesa porque soy un microbiólogo evolutivo que estudia el vínculo entre la transmisión y la evolución de las enfermedades infecciosas. En particular, dedico mucho tiempo a considerar los efectos de las vacunas en la evolución de patógenos y los efectos de la evolución de patógenos en el impacto de las vacunas.
La nueva versión de SARS-CoV-2, llamada linaje B.1.1.7, se está extendiendo en el Reino Unido y posiblemente más allá. Las diferencias entre el virus antiguo y el nuevo incluyen 23 mutaciones en el código genético del virus que han alterado cuatro proteínas virales.
Ocho de estas 23 mutaciones afectan la proteína de pico. Esto es importante porque la proteína de pico permite que el virus ingrese a las células humanas y es un objetivo clave de nuestra respuesta inmune, tanto para combatir el virus durante la infección como para protegernos de enfermedades después de la vacunación con las vacunas Pfizer y Moderna.
Si los cambios en la proteína de pico ayudan al virus a entrar más fácilmente en las células humanas, entonces el virus podría transmitirse de persona a persona más fácilmente.
Estas mutaciones también pueden alterar qué tan bien el sistema inmunológico del huésped combate el virus, reduciendo potencialmente la eficacia de las vacunas actuales.
¿Qué tiene de diferente esta nueva versión de SARS-CoV-2?
Las muestras del nuevo virus aislado de pacientes sugieren que esta variante ha aumentado en frecuencia relativa durante los últimos tres meses.
El aumento en la frecuencia es preocupante, ya que sugiere, pero no prueba, que las cepas B.1.1.7 de SARS-CoV-2 son más transmisibles que el virus original. Algunos han estimado que el nuevo virus puede ser hasta un 70% más transmisible que el antiguo. Si bien estas estimaciones son consistentes con los datos, es demasiado pronto para llegar a una conclusión definitiva.
Si se confirma este aumento en la transmisibilidad, podría deberse a que las mutaciones en la proteína espiga le permiten unirse más estrechamente al receptor ACE2, que proporciona una puerta de entrada para que el virus ingrese a las células humanas.
Pero también podría deberse a cualquiera de los otros cambios en el virus.
¿Es más peligroso? Si es así, ¿por qué?
Si la nueva versión, B.1.1.7, es más transmisible que el virus antiguo, será más peligrosa en el sentido de que enfermará a más personas.
Sin embargo, no conozco pruebas sólidas de que exista alguna diferencia en la gravedad de la enfermedad causada por la nueva versión de este virus en comparación con la anterior. Dicho esto, con tan pocos casos conocidos, es posible que aún sea demasiado pronto para decirlo.
¿Seguirán siendo eficaces las vacunas Pfizer y Moderna contra esta nueva cepa?
Tanto las vacunas Pfizer como Moderna funcionan capacitando a nuestro sistema inmunológico para reconocer una versión específica de la proteína de pico viral. La versión de la proteína de pico utilizada por las vacunas se diseñó para coincidir con la del virus antiguo, no con la del virus B.1.1.7. Esto significa que las vacunas podrían volverse menos efectivas de lo esperado si este nuevo virus se propagara ampliamente.
El desajuste entre la vacuna y el virus es un desafío continuo para los científicos encargados de desarrollar la vacuna contra la influenza estacional. Pero incluso con una discrepancia entre el virus y la vacuna, la vacuna contra la gripe reduce la probabilidad y la gravedad de la enfermedad.
Por lo tanto, la pregunta no es si las vacunas serán efectivas, sino cuán efectivas serán. La gravedad del desajuste es importante, pero la única forma de determinar su impacto en este caso es mediante un estudio científico y, que yo sepa, aún no se han recopilado datos al respecto. En otras palabras, es demasiado pronto para decir si esta nueva variante influirá en la eficacia general de las vacunas Pfizer y Moderna y cómo lo hará.
¿Debería la gente recibir la nueva vacuna de ARNm?
La aparición de este nuevo B.1.1.7 hace que sea aún más importante que las personas se vacunen lo antes posible.
Si esta nueva versión es más transmisible, o si la vacuna es menos efectiva debido a un desajuste entre el virus y la vacuna, será necesario vacunar a más personas para lograr la inmunidad colectiva y tener esta enfermedad bajo control.
Además, ahora tenemos pruebas de que la proteína de pico de SARS-CoV-2 puede cambiar drásticamente en poco tiempo, por lo que es fundamental que tengamos el virus bajo control para evitar que evolucione más y socave por completo los esfuerzos de vacunación.
