El Departamento de Servicios de Salud de Wisconsin (DHS, por sus siglas en inglés) emitió una alerta de salud este miércoles con respecto a la primera muerte pediátrica en los Estados Unidos posiblemente relacionada con el misterioso brote de hepatitis pediátrica y adenovirus, según un comunicado reciente.
«Desde que se le notificó sobre este grupo de hepatitis asociado con adenovirus, el DHS ahora está investigando al menos cuatro casos similares entre niños en Wisconsin. Esto incluye dos niños que tuvieron resultados graves, un trasplante de hígado y una muerte».
La declaración insta a los médicos en los Estados Unidos a considerar la posibilidad de realizar pruebas de adenovirus en pacientes pediátricos que tienen hepatitis de etiología desconocida y reportar estos casos a los laboratorios de salud pública de sus estados y a los Centros para el Control y la Prevención de Enfermedades (CDC).
La agencia emitió por primera vez una alerta de salud oficial la semana pasada con respecto a un grupo de 9 casos pediátricos previamente sanos con enfermedad hepática, incluidos tres con insuficiencia hepática y dos que requirieron un trasplante de hígado, que ingresaron en un gran hospital infantil en Alabama entre octubre de 2021 y febrero. 2022. El Departamento de Salud Pública de Illinois está investigando tres casos sospechosos de hepatitis en menores de 10 años posiblemente relacionados con el adenovirus, incluidos dos en los suburbios de Chicago, donde uno requirió un trasplante de hígado.
La Organización Mundial de la Salud (OMS) reportó 169 casos de hepatitis aguda de etiología desconocida, con edades comprendidas entre 1 mes y 16 años, con aproximadamente el 10 % que requirió trasplante de hígado al menos una muerte, según un informe del 23 de abril.
La organización señaló que, hasta el momento, la mayoría de estos casos se encuentran en el Reino Unido, donde el país «… ha observado recientemente un aumento significativo de infecciones por adenovirus en la comunidad (particularmente detectadas en muestras fecales de niños) luego de niveles bajos de circulación a principios de la pandemia del COVID-19».
La OMS dijo que 74 casos dieron positivo por adenovirus en el brote que ahora ha llegado a 12 países en todo el mundo. Desde su informe, Japón y Canadá ahora están investigando casos similares, según USA Today.
La organización también señaló que la «gran mayoría» de los casos informados no recibieron la vacuna COVID-19.
«Aunque actualmente el adenovirus es una hipótesis como causa subyacente, no explica completamente la gravedad del cuadro clínico. La infección con el adenovirus tipo 41, el tipo de adenovirus implicado, no se ha relacionado previamente con una presentación clínica de este tipo», dijo la OMS.
